Tulsidas Gwalani MD 
Founder and Chair 

Dr. Tulsidas Gwalani is a highly experienced Interventional Pain Management and Physiatry physician with a long-standing commitment to helping patients restore function, reduce pain, and improve overall quality of life.

He completed his residency training in Physical Medicine and Rehabilitation at Temple University in Philadelphia, where he developed a strong foundation in comprehensive, function-focused patient care. Dr. Gwalani went on to pursue advanced fellowship training in Interventional Pain Management at California Pacific Medical Center, where he trained under the renowned Dr. Elliott Krames, a global leader in neuromodulation and interventional pain therapies.

Dr. Gwalani is Board Certified in Physical Medicine and Rehabilitation and specializes in interventional pain treatments that emphasize non-surgical solutions for spine, joint, and nerve-related conditions. His clinical approach integrates precision-based interventions with a deep understanding of functional rehabilitation, allowing him to tailor treatment plans to each patient’s individual goals and needs.

In 2003, Dr. Gwalani founded the practice with a vision of delivering patient-centered, high-quality pain care grounded in innovation, integrity, and long-term outcomes. As the founder, he has remained deeply involved in shaping a care model that prioritizes thoughtful evaluation, evidence-based treatment, and compassionate physician-patient relationships.


Jaimish Gwalani MD

Jaimish Gwalani MD
Founder & Director 

Dr. Jaimish Gwalani is an Interventional Spine and Pain Management physician dedicated to helping people manage their pain through pain treatments. He grew up in the San Francisco Bay Area and went on to pursue further training on the East Coast at Jefferson Medical College in Philadelphia. After which, he pursued residency training at the University of California, Davis Medical Center.  He stayed on to pursue additional training under the Interventional Spine and Pain Management Fellowship. The program is just one of four in the country to have earned a 2016 Pain Medicine Fellowship Excellence Award from the academy. He is Board Certified in Pain Management by the American Board of Pain Medicine.

He specializes in interventional spine care, which includes joint injections, nerve blocks, spinal cord stimulation therapy, and epidural procedures among the many treatments offered. His goal is to provide the best care he can for each patient and he truly believes this starts with listening to them to develop the best treatment plan. He aims to challenge the norms by exploring non-surgical treatment options that exceed peoples’ expectations.


Priyanka Gwalani MD

Dr. Priyanka Gwalani is an Internal Medicine physician with advanced training in Pain Medicine, focused on helping patients manage pain, improve function, and return to daily activities with greater comfort and confidence.

She completed her residency training in Internal Medicine at Virginia Commonwealth University, where she developed a strong foundation in comprehensive medical care and complex chronic disease management. Following residency, Dr. Gwalani pursued specialized training in Pain Medicine at Mission Pain Treatment Center, gaining hands-on experience in a wide range of pain related conditions

Dr. Gwalani treats a broad spectrum of pain and musculoskeletal conditions, including chronic neck and low back pain, joint pain, and neuropathic pain, among others. Her clinical practice includes interventional and non-interventional treatments. 

She is committed to providing patient-centered care rooted in careful listening, clear education, and individualized treatment planning. Dr. Gwalani emphasizes non-surgical, evidence-based approaches designed to reduce pain while improving mobility and overall quality of life.


Dr. Alexander Sable-Smith

Dr. Sable-Smith is a triple board-certified physician with advanced training in pain medicine, palliative medicine, and family medicine. He brings a comprehensive, patient-centered approach to the evaluation and management of complex pain and symptom conditions.

He completed his residency training in Family Medicine at the University of Colorado, where he developed a strong foundation in longitudinal patient care and whole-person medicine. He subsequently pursued fellowship training in both Palliative Medicine and Pain Medicine at Stanford University, one of the nation’s leading academic medical centers. Dr. Sable-Smith continues to teach medical residents and fellows at Stanford, remaining actively involved in physician education and mentorship.

Dr. Sable-Smith specializes in multidisciplinary, team-based pain and symptom management, integrating interventional pain techniques with medical, functional, and supportive care strategies. His clinical expertise includes interventional pain management as well as complex medical decision-making for patients with chronic pain and serious illness.


Haramrit Hansra MD

Dr. Haramrit S. Hansra, MD is a board-certified Interventional Radiologist with specialized expertise in minimally invasive, image-guided procedures that play an important role in modern pain management.

Dr. Hansra completed his medical training with a focus on diagnostic and interventional radiology and is Board Certified by the American Board of Radiology in both Diagnostic Radiology and Interventional Radiology. 

In addition to his broad interventional radiology practice, Dr. Hansra has developed particular expertise in pain-focused embolization procedures, including genicular artery embolization (GAE) for chronic knee pain related to osteoarthritis and shoulder artery embolization for refractory shoulder pain. These advanced, non-surgical techniques are designed to reduce inflammation and pain by selectively targeting abnormal vascular supply, offering patients meaningful symptom relief with minimal downtime.

Dr. Hansra works extensively to provide comprehensive, patient-centered care. His approach emphasizes precision, safety, and collaboration, helping expand non-operative treatment options for patients who wish to avoid surgery while improving function and quality of life.
